When we were approached to give an Energy Audit to this particular house, we knew that it was huge and had huge air infiltration issues. We and the Homeowners decided that the best bang for their buck would be for us to access their knee walls on the third floor and insulate their rim joist. After we cut a couple of holes and removed the sub-flooring, we proceeded to cut individual squares of 2" expanded foam to fill each floor cavity, and sealed each edge with cans of a closed cell expanding foam. When this was completed, the floor was warm, and the rest of their house was able to maintain a constant temperature ( that wasn't freezing.) 
After we finished with the crawl spaces, we took the same approach to their fireplace. You could see the sun shine in through the cracks between the bricks and drywall, so we foamed in the cracks, installed a vapor barrier, framed in, insulated, and sheet rocked the whole thing. Incredible difference. 
After we addresses the air infiltration issues, we fired up the blower door, and found all the tiny spots that we missed the first time. Now their house is tight! We then moved into the main bathroom, where the homeowners were having a moisture problem on one wall. What we found when we tore into it was a whole world of black mold! The entire shower and hot tub surround had to be demolished and rebuilt. We then built it back together and did a glass tile on it instead of the old ceramic tile, and got a very modern looking new bathroom.
